Dear Claim Review Board:

I am writing to appeal the decision to deny my request for a waiver of overpayment.

The overpayment was not my fault and I cannot afford to pay the money back because
I have to pay bills for food, clothing, housing, medical care, and other necessary expenses.

I’m enclosing copies of mortgage, credit card and car payments, and receipts of food,
clothes and medication.

I am requesting that you consider this appeal to waive the collection of overpayment.

Thank you.

Sincerely,

Jane Doe

Dear The Appeals Committee:

This is to appeal the decision to deny my request for extension to remove the condition in English.

Since my father passed away in February, I have had trouble concentrating on my course work. As a result, I failed the third English Placement Test.

Since then, I have been to counseling to deal with my grief. Now, I have overcome it and am ready to take the English Placement Test again.

I hope you reconsider my request for extension to remove the condition in English.

Thank you.

Sincerely,

Jane Doe
