Semester Goals

Place a check next to the topics that you and your student think he/she will need to focus on during the semester.  Record the dates that you worked on each topic.  Every three weeks, you and your student should rate the progress that the student has made in each topic. 
 

1 = no improvement
2 = little improvement 
3 = some improvement 
4 = good improvement 
5 = mastered this topic!
